Meitheal Product Verification Request
A Product Verification request may be initiated by an Authorized Trading Partner in physical possession of the Meitheal product which is the subject of the request.
Please follow these steps to initiate a verification request :
Download and complete this form. All fields denoted with an asterisk (*) are required.
- If possible, take photos of the product label, including name, strength, LOT, EXP, GTIN, SN and the 2D Matrix Code.
- Attach the completed form and any photo(s) to an email and send them to us at product.verification@meithealpharma.com
- We will acknowledge receipt of your verification request. If further information is required, we will contact you.
Meitheal will investigate the product identifier and respond to your request for verification by determining whether the product identifier provided corresponds to the product identifier imprinted by Meitheal.
